Below you will find important financial aid forms, including those related to Federal Verification, for the 2025-2026 and upcoming 2026-2027 academic years. 

Federal Verification

If you have been asked to submit additional information for Federal Verification, please be sure to check Student Requirements and Active Messages. Please Do Not Submit Documents Until They Are Requested by UNCW.

Completed and signed forms (aside from DocuSign) should be uploaded via Fax: (910) 962-3851 or through the FAFSA & Eligibility Documents portal via mySeaport. Please do not e-mail.

You are not required to use the DocuSign/E-Signature option that is offered on some of the forms below. If you prefer to print and provide a handwritten signature, you are more than welcome to submit the documents in that format instead.
