Forms for Current Graduate Students:
Thesis and Dissertation:

- DocuSign form is initiated by the student then electronically signed by the advisor

- DocuSign form initiated by the student that specifies the electronic release information for the thesis or dissertation
Withdrawal and Leave of Absence:
- U.S. Service Members
- Students who are called to active duty, mobilization, or deployment should contact the Office of Military Affairs with their official orders and can receive guidance on next steps.

- DocuSign form initiated by student, that should be used when the student is withdrawing from the program and the university, with no plans to return.

**Students who withdraw from all courses during a semester should complete a Leave of Absence form to be eligible to continue with course work at a later semester.** - (Regular Term Instruction-RTI programs)
